To find the resident survey

  1. Go to www.ohiorecoveryhousing.org
  2. Click on Resources for Operators
  3. Click on Outcomes Tools
  4. Click on Find the Outcomes Tools Surveys
  5. Click on Resident Survey
  6. Enter password: recovery-ohio
