Self Eligibility Screening
We highly recommend that you complete this self screening assessment to determine whether the supportive housing that you provide generally appears to be a "recovery residence" or "recovery housing" before investing significant time in completing the application. A warning will appear at the bottom of this page, if you answer one or more of the following questions in a way that would indicate that you do not operate a recovery home or recovery residence. Regardless of whether you get a warning or not, there is no guarantee that your application will result in certification.
